Rapamycin Dosage and Side Effects in Pets: A Practical Guide

Short answer: Longevity dosing of rapamycin is low and intermittent — a fraction of transplant dosing, given weekly in cats and around three times weekly in dogs, scaled to body weight. At these doses the commonly reported side effect is mild, transient stomach upset. Diabetes, active infection, significant liver disease and planned surgery are the main exclusions. Every protocol requires veterinary review, because dose, interactions and monitoring are the parts that make it safe.

Why the dose is the whole story

Rapamycin does two quite different things depending on how it is given.

Given daily at high dose, it suppresses the immune system — the effect wanted in transplant medicine, where the goal is preventing organ rejection.

Given intermittently at low dose, it partially quiets mTOR without sustained immunosuppression. Research at these lower, pulsed doses has reported improved immune responses in aged animals rather than blunted ones — in vaccine response studies, for instance.

Same molecule, opposite practical profile. When people worry that rapamycin "suppresses immunity," they are almost always thinking of the transplant regimen.

How dosing is calibrated

Body weight is the starting point — a 4 kg cat and a 60 kg mastiff are not on the same milligram dose.

Species and schedule differ. Cats are typically dosed once weekly using a delayed-release, acid-resistant formulation, at strengths stepped by body weight. Dogs are typically dosed around three times weekly.

Formulation matters. A delayed-release coating protects the molecule through stomach acid. Flavoured oral suspensions exist for cats that refuse tablets, which matters more than it sounds — the best protocol is the one that actually gets administered.

Baseline bloodwork comes first, and periodic rechecks continue afterwards.

Side effects actually reported

At longevity doses, most animals show none. What is reported:

  • Mild gastrointestinal upset in the first weeks — the most common by a wide margin, usually transient, often improved by giving the dose with food.
  • Mouth ulcers — recognised with mTOR inhibitors, uncommon at low intermittent dosing.
  • Delayed wound healing — the practical reason protocols pause around surgery.
  • Metabolic changes — shifts in blood lipids or glucose regulation are a known class effect and are among the reasons for periodic bloodwork.

Serious adverse effects at longevity dosing are uncommon in reported veterinary experience, but "uncommon" is not "impossible," which is why monitoring is not optional.

Who is not a candidate

  • Diabetes or impaired glucose regulation — mTOR inhibition can affect glucose handling.
  • Active or recurrent infection — treat the infection first.
  • Significant liver or kidney disease — affects metabolism and clearance.
  • Planned surgery — pause before and after.
  • Pregnancy or nursing.
  • Certain concurrent medications — see below.

Drug interactions worth naming

Rapamycin is metabolised through the CYP3A4 enzyme system. Drugs that inhibit that system — including some antifungals such as ketoconazole and itraconazole, and some antibiotics — can raise rapamycin levels. Drugs that induce it, including some antiseizure medications, can lower them.

What monitoring looks like

Bloodwork before starting, establishing the baseline. A recheck in the first months. Then periodic monitoring, adjusted to the animal's age and health status. For cats on a cardiac protocol, echocardiography tracks the thing that actually matters. Weight is tracked throughout, because unexplained weight change is an early signal in many conditions.

Owners are asked to report appetite changes, persistent vomiting or diarrhoea, mouth sores, and lethargy that does not resolve.

The honest summary

The safety profile of rapamycin at longevity doses is reassuring but not trivial, and it depends entirely on the protocol being run properly: right dose for the weight, right schedule, screened contraindications, checked interactions, monitored bloodwork. That is the difference between a medical protocol and buying a molecule — and it is why access without oversight is the wrong model for this drug.
