How Is HCM in Cats Treated? The Honest Map of Every Option

Short answer: there is still no medicine licensed to treat feline HCM itself — and yet the treatment landscape has changed more in the last three years than in the thirty before. What exists today: proven clot prevention for at-risk cats, effective heart-failure management when the disease decompensates, and — new — two research-stage therapies aimed at the disease itself: rapamycin for slowing wall thickening, and myosin inhibitors for relieving obstruction. What a cat should receive depends entirely on its stage and form, which is a cardiologist's call.
First: what stage, what form?
HCM treatment is not one decision; it is a map with two axes. Stage: subclinical (thickened wall, no symptoms) versus clinical (heart failure or a clot event). Form: obstructive versus non-obstructive — whether the thickened wall narrows the outflow while the heart contracts (the distinction explained here). Every option below applies to some squares of that map and not others.
What has solid evidence
Clot prevention (clopidogrel). The most confident prescription in feline cardiology. Cats with enlarged left atria are at risk of arterial thromboembolism — the clot that paralyses the hind legs — and the randomised FATCAT trial showed clopidogrel clearly superior to aspirin at preventing recurrence. At-risk cats are typically on it for life.
Heart-failure management. When fluid accumulates, diuretics (furosemide) relieve it, often dramatically. This is symptomatic treatment — it manages the consequence, not the muscle — and it works; many cats gain good months to years on it. Oxygen and drainage of chest fluid carry the crisis phase.
What is prescribed but weakly supported
Beta-blockers (atenolol). Long the reflex prescription for a murmur. The honest evidence: in the controlled study of subclinical cats, atenolol did not improve outcomes, and routine use in asymptomatic HCM is no longer supported by data. It retains narrower uses at a cardiologist's discretion. If your cat left a routine check-up with a beta-blocker and no echocardiogram, the missing item is the echo, not a second drug.
The new front: therapies aimed at the disease
Rapamycin — at the disease's course. A delayed-release formulation studied in subclinical HCM was associated with slower progression of wall thickening against placebo, and holds the first conditional FDA approval in this disease. Myosin inhibitors — at the disease's mechanics. Mavacamten, fully approved for obstructive HCM in human medicine, is investigational in cats: early studies show reduced outflow obstruction after dosing. Only for obstructive cats with preserved systolic function, only under echo-guided titration. The explainer: mavacamten for cats; the therapy: Mavacor.
Neither is a cure, and any page telling you otherwise is selling ahead of the evidence — including pages of ours, if we ever wrote one, which is why we do not.
What every HCM cat needs regardless
Scheduled echocardiography — the disease is staged and every drug decision is made from that image. A weekly sleeping breathing count at home (under ~30 breaths/minute; a sustained rise is the earliest failure signal). And an owner who treats sudden hind-leg pain or laboured breathing as the emergencies they are.
The honest summary
Managed HCM in 2026 looks like this: clopidogrel where clot risk exists, decisive diuretics if failure comes, no reflex beta-blockers, and — for the first time — a real conversation with your cardiologist about disease-directed therapy: rapamycin for the course in subclinical cats, mavacamten for the mechanics in obstructive ones. The map has more roads than it did. A cardiologist still has to drive.
